 When this application is signed (or stamped or coded with a number) by the Mechanical Inspector and the Permit Number is affixed, This form becomes the  permit.  

GENERAL: Mechanical work shall not be started until the application for permit has been filed. All installations shall be in conformance with the State Mechanical Code. No work shall be concealed until it has been inspected. The telephone number for the inspector is provided on the permit form. When ready for an inspection, call the inspector providing as much advance notice as possible. The inspector will need the job location. This request should be made by the contractor of record.  It is the contractor of record's obligation to provide access at the inspection site.  A key, person on site, or arrangements for access should be made in advance.  A fee of $50 per visit may be charged if access is not provided and an inspection cannot be made.

EXPIRATION OF PERMIT: A permit remains valid as long as work is progressing and inspections are requested and conducted. A permit shall become invalid if the authorized work is not commenced within six months after issuance of the permit or if the authorized work is suspended or abandoned for a period of six months after the time of commencing the work. A PERMIT WILL BE CANCELLED WHEN NO INSPECTIONS ARE REQUESTED AND CONDUCTED WITHIN SIX MONTHS OF THE DATE OF ISSUANCE OR THE DATE OF A PREVIOUS INSPECTION. CANCELLED PERMITS CANNOT BE REFUNDED OR REINSTATED. This is important and is enforced.

Notes: ITEM #2, RESIDENTIAL HEATING SYSTEM: This item is used for the installation of a heating system in a new residential structure. Items # 8, #10, and #15, SHOULD NOT BE charged again. Replacement systems should be itemized. ITEMS #13 is for LP vendors only.  Hoods for commercial use: Should be listed under item #22 as unlisted equipment for each 6 foot of length.  Wood/fuel fired yard boilers are not authorized in the City of Bangor.
